I already have a Hospice nurse, what benefit would having a Graceful Transitions Doula provide?

Hiring a Graceful Transitions Doula is an added support to your Hospice and medical care team. A Graceful Transitions Doula can help to fill in additional needs for the family / client when a Hospice nurse is not available to be at bedside. A Hospice nurses time spent in the home is often limited and pre-set. We can provide an added layer of non-medical support during times you may want or need additional support.

 

I have met with a Lawyer and I have a power of attorney or medical power of attorney, what additional service does Graceful Transitions provide?

We provide personalised education to you and your selected Power of Attorney or Medical Power of Attorney. We have found that in times of crisis POA/ MPOA’s are asked to make very detailed decisions on hospital level care. Your POA/MPOA may not be in a frame of mind to fully understand medical terminology or therapy/treatment options when a crisis occurs. Our goal is to educate your POA/MPOA and our clients ahead of time. Their focus and attention at that moment should be on being “present” and not using “Google” to quickly understand medical terms and treatment/therapy choices.

 

My loved one needs to go into a nursing home/ rehab facility, now what?

Selecting a facility to allow our loved one to go to is a huge decision and one not to be taken lightly. Graceful Transitions can assist you with evaluating facilities. We can provide a checklist of questions to get answered, and share our expertise and experience in this area. We also offer services to physically accompany you or your loved one on a “ tour” of any potential facility you may be considering.

 

I am not medical and do not understand hospitals- nursing homes- hospice- palliative care. I want to make sure I understand my choices.

This is an area where Graceful Transitions can be of great support and education. We provide individualised education to help you understand the basics of hospital care, nursing homes, end of life choices you or a loved one may be faced with. We are are there to partner with you to support your needs and answer your questions.

I’m not ready to talk about dying. I’m young and good health, what benefit does scheduling a session with a Graceful Transitions doula bring ?

If you are over the age of 18 you should have someone formally named to speak on your behalf if you for some reason you could not. As we age and our parents and grandparents age, we all need to be aware of end of life choices we may be asked to make for a loved one in the future. Our goal is to provide education and promote open discussions long before a crisis arises. Education about end of life is the key to helping to bring a sense of “Calm” to what may feel like an out of control and unexpected situation.
